This form should be completed by any Fraternal Order of Police member or civilian to provide information, ideas, opinions and/or any pertinent facts (negative or positive) about all persons currently in public office, being considered for public office, or any person actively seeking public office. Examples: City Council Members, Judges, Presidents, Senators, Representatives, Clerks, Prosecutors, Coroners, Sheriffs, Treasurers, Clerk of Courts, Recorders, etc.

This form should be completed if any member or civilian witnesses or is made aware of anything the 1900 Club should know to show what kind of person we are considering for an endorsement.
