This form should be completed by any police officer who is the victim of, or who witnesses, or is made aware of any disrespect, discourtesy, procedural violation, ethical violation, or any violation of any law by the City of Cincinnati Citizen Complaint Authority or an employee of the City of Cincinnati Citizen Complaint Authority.
